The GLP-1 reality

The medication is doing its job. Most prescribers aren't doing theirs.

GLP-1 weight loss medications work. The problem isn't the drugs. The problem is what's getting skipped around them.

Research on semaglutide and tirzepatide shows that a large share of the weight lost on these medications is lean mass, not fat. In the major trials it runs around 25%, and in some studies as high as 40%. That lost lean mass is muscle, and the same under-supervised pattern puts bone density and connective tissue at risk too. Most prescribers don't program the protein, the resistance training, or the supplementation that protects what matters. Most never mention what happens when you come off.

The data on what happens next isn't pretty. About two-thirds of the weight lost on these medications returns within a year of stopping. The fat comes back. The muscle and bone don't come with it.
